How did supervised visitation start out with your child and child's father?

Varies a lot. For my cousin, the supervised visitation was done by the grandmother, the father's mother. So, the mom brought the child to the grandmother's home and she was required to always be present when the child was in contact with the father. As time went on, she just had to be present in the home. In this particular case, the grandmother always had regular contact with the child so there weren't any concerns by the mother over proper care.
